Registering a patient

Create a patient record with basic contact and demographic information.

Registering a patient is the first step to creating a medical record and beginning treatment. You'll enter the patient's name, age, contact details, and other identifying information so the clinical team can track their medical history and provide accurate care.

Before you start

  • You have Contacts permission (usually assigned to front-desk or administrative staff)
  • Have the patient's full name, date of birth (or age), and contact information ready
  • Decide if this is an adult or pediatric patient

Steps

  1. Go to Contacts → Patients
    Open in Usystems

  2. Click New Patient or the + button in the top-right corner

  3. Enter the patient's Full Name (required)
    This appears on all medical records, prescriptions, and invoices

  4. Enter Date of Birth or Age
    Usystems uses this to calculate the patient's current age on medical forms

  5. Enter Email and Phone (optional)
    Used for appointment reminders and contact purposes

  6. In the Address section, add the patient's full residential address

  7. Optionally, enter Emergency Contact name and phone number for critical situations

  8. Click Save

The patient record is now created and ready for a first appointment or consultation.

Tips & common mistakes

  • Name accuracy matters: Use the full legal name as the patient will recognize it. This helps avoid confusion when multiple patients have similar names.
  • Age or date of birth: Usystems calculates age from the date of birth automatically. If you only have age, you can estimate the birth date.
  • Edit anytime: Patient details can be updated at any time without affecting existing medical records.
  • Phone number format: Enter numbers in your country's standard format; Usystems stores them as entered for easy recall.
